Failed to clean up telephony adapter devices
As part of the Blueworx Voice Response startup procedure, or when Blueworx Voice Response shuts down, Blueworx Voice Response makes sure that everything is cleaned up. Remnants of any logical devices are removed at this point. The DTQA_destroy_adapter program is used to make sure that logical devices associated with the telephony device are removed correctly. This program failed. The I/O subsystem cannot be started.
Try restarting the pSeries system unit. Try reseating or removing certain cards to see if the problem follows a particular adapter. If the problem persists, call Blueworx Support.
